Synchronous esophageal squamous cell carcinoma and gastric adenocarcinoma.

چکیده
úlcero-infiltrante em antro gástrico, circunferencial até o piloro, sem estenose, a mais de 10 cm da junção esofagogástrica. A biópsia da lesão esofágica diagnosticou carcinoma escamoso de esôfago, e a gástrica adenocarcinoma gástrico. Tomografia de tórax, abdome e pelve descartavam derrame pleural ou adenomegalia torácica. Mostrava espessamento parietal concêntrico do terço inferior do esôfago, junção esofagogástrica, da região antropilórica e linfonodomegalia junto à pequena curvatura gástrica. Fígado estava com dimensões e contornos normais, sem sinais de lesões focais. O paciente foi submetido à esofagogastrectomia total trans-hiatal com linfadenectomia e reconstrução com o cólon esquerdo e jejunostomia. A histopatologia confirmou presença de carcinoma epidermóide de esôfago médio de 7,5 x 6,5cm com margem proximal de 4,5 cm e adenocarcinoma gástrico Borrmman III de 4,0 x 2,5cm com margem distal de 2,0 cm (Figura 1) 2). Foram isolados ao total 60 linfonodos, dos quais dois eram positivos para carcinoma epidermóide (um no grupo cárdico direito e um no esquerdo) e dois eram positivos para adenocarcinoma na pequena curvatura. A classificação patológica para tumor esofágico foi de pT3pN1 e para o tumor gástrico pT4apN1a.
